Alissa Perrucci's book is an invaluable resource for the counseling, family planning, and reproductive justice community. She thoughtfully weaves clinical evidence with demonstrative examples and constructive exercises. As a nurse practitioner with a background in counseling, I found the book to be both a succinct review of the principles of good counseling, as well as a concise framework for assessing patients needs and assisting them through their decision-making process. The book contains excellent tools to help counselors clarify their personal values, as a means to prevent them from interfering with the delicate skill of supportive, nonjudgmental counseling. I am so happy to add this book to my library as a critical resource for options counseling, abortion care and women's health.
Dr. Perrucci has written an invaluable book. The frameworks she builds for exploring a patient's belief systems focuses on the important work of abortion counseling but also transcends to all discussions health care providers have with patients making difficult choices: chemo or surgery? Hospice or treatment? At its core, this book is about helping the provider become a guide for the patient, allowing her space to tell her story, voice and resolve conflict, ensure her support throughout her decision.The beauty of this book is that it truly demystifies the process of supporting the patient. She literally walks you through exercises that break down different levels on which to respond to a patient when she has emotional, spiritual or moral concerns about her decision. Shortly after finishing this book, I had a patient come in for a follow up appointment after her abortion and she started to cry. Instead of just saying comforting words and offering her a tissue, as I previously would have done, I now had the tools to explore her feeling with her, the origins of her feelings and consequently was able to help reframe her vision of herself as a good person who made a moral decision that was the best for her and her family. That is the power of this book; it is a tool to help women see the difficult choices they make as moral choices and themselves as good people. In a highly contentious political landscape rife with shaming women who have had abortions, this is an incredibly powerful transformation.An unexpected gift of this book is helping me reframe my own work as an OBGYN in training and a future abortion provider. In the first chapters of the book, Dr. Perrucci discusses the importance of saying the word "abortion." Not "the procedure," not dancing around it, but "abortion," in the same tone which you would say "contraception" or "cholecystectomy." Normalizing this procedure, which is one of the most common for women in the US, is incredibly important and powerful for us as providers as well. Another powerful aspect of this book for me was the opportunity to examine our own biases as providers. What do you think about an abortion for a fatal anomaly? Does that change if it's not? If the patient is a teen? A mom? If she declines contraception afterwards? The options we offer in our clinics, from abortion, to parenting, and all array of contraception - including none! -- are really important to authentically offer if we are to be true champions of choice. This book offers a robust toolkit for examining all of these dimensions with the patient as our focus.
As a primary healthcare provider for women, this book has helped me tremendously. Certainly, it has transformed my counseling of women and adolescents with unintended pregnancies. Rather than presenting positive results apprehensively, I now find myself doing so confidently, methodically, feeling open and thoroughly supporting women's diverse decisions (including those I would not make myself and had been lukewarm about before reading this). When I began reading, I had already spent a decade professionally providing health counseling and doubted I could improve much, though I was interested in the topic. To my chagrin, the book made plain my hidden biases and counseling shortcomings and taught me how to deal with them so that I could better serve my patients. Considering that half of all pregnancies in the U.S.--and even more than that among my underserved patients--are unintended, this book is essential for all healthcare providers serving women. It is thoroughly engaging with case studies that exemplify phrasing and nuanced counseling strategies, comprehensively researched, illuminating about the informed consent process for all medical decisions, and cuts through the social stigma surrounding abortion while reckoning with the reality of the experience of stigma. Unexpectedly, it has vastly strengthened my ability to engage in shared decision-making with patients facing difficult medical choices of all sorts. I was so grateful I had read the chapter on Decision Assessment the same week that a woman in her 50's presented with abnormal heart sounds after not taking her regular heart medications. Using the counseling techniques Dr. Perrucci masterfully presents in that chapter, I felt I could more actively listen to the patient's circumstances, allow her to cry when she realized the seriousness of the situation, and better prepare her for the life change and series of decisions her new symptoms would likely necessitate. She was ultimately diagnosed with congestive heart failure in the emergency department (where I'd sent her). When she returned the following week with her family, we were both much more prepared to face the medical challenges ahead due to the counseling groundwork we'd already begun. This effect of reading this book was unexpected but not surprising: Dr. Perrucci's book is profoundly insightful, instructive and its many nuanced lessons transform situations that would otherwise be excruciating for patient and provider alike into cathartic, pivotal and often rewarding healthcare partnerships. This is a counseling reference to which I will undoubtedly turn for the rest of my career.
